- Soil Permit (pdf; opens in new window): MUST accompany all samples from outside the continental United States. Prevents these samples from being confiscated and destroyed by U.S. Department of Agriculture inspectors. Certifies that Soil Foodweb Oregon is a facility permitted to accept samples from outside the United States and is capable of properly handling such samples. ALWAYS use this form when sending in samples from outside the contiguous 48 States. In particular, Hawai'i, Puerto Rico, and other non-contiguous areas of U.S. territory are considered "foreign" by the Department of Agriculture.

Pricing Policy
- All samples submitted for testing must be paid for at the time services are performed.
- We accept Check, Credit Card (Visa, MasterCard, American Express), Wire Transfer, and PayPal.
Please note that we can no longer offer a discount for prepayment.
